We now have a permanent position for a Care and Support Worker to join our team at Poppy Court in Coventry. The Extra care scheme provides 24 hour care and support to customers aged 55 years and over. Every customer has an individual care plan. We have a dedicated team of staff who are trained to provide a high standard of care.
You will be working within our Older Persons Services, offering a person centred care and support to customers based on their individual risk and needs assessment and care and support plan, both devised with the customer to promote choice, independence and empower the individual to live a fulfilled and active life.
As a Care and Support Worker, you will provide customers with a quality service to maintain an independent lifestyle. You will be responsible for assisting customers with medication and liaising with other professionals to support the customer’s health and wellbeing.
You will also be participating in the scheme’s Activity Planner, providing a range of social activities to our customers.
We are looking for a team player, with first class communication who has a real passion for delivering high quality care. We can provide full training to the successful candidate.
